The Last of Us.
The companion AI can be genuinely awful. I understand that they had to start from scratch 5 months before release, and 90% of the time it's fine, but the other 10%... There's something about Ellie sprinting back and forth between two pieces of cover in-front of several Clickers that really tugs an your immersion :p
