The vampire boss battle in Illusion of Gaia. That fight was a freaking nightmare! I didn't bother counting my deaths, but It had to be within the range of 50-100. The fight took me well over two hours, and a lot of my deaths were really quick.
Second would probably be the Imlerith boss in the Witcher 3, though I guess that's discounted cause I'm on Death March.
