In first world countries it is actually winter which kills more. People have had a good job of avoiding deserts, while harsh winters happen in most first world countries (in the temperate zone.)

Also the IPCC has found rising CO2 levels to have a net increase in vegetation growth, especially in the tropics. Other climatologists have also found it to be responsible for something like 14% greater agricultural output in the world's breadbaskets than if CO2 we're to remain constant.
