You never have to kill everyone, you can get pass the whole game only using CQC and the tranquilizer gun, if you're spotted you can always regain stealth, if you can't its because you suck.
You can actually pass through the whole game doing a non lethal run, just like in MGS2.
And you very much can open doors slowly.
