I've beaten all the home console games minus the first one and Peace Walker on Extreme (Not E-Extreme, though). I've almost collected all the dog tags in MGS2, I'm missing the Easy and Very Easy playthroughs. Got all animals for the Markhor title in MGS3. Got the bandana and stealth camo in MGS4 for not entering alert mode and not killing anyone during the playthrough. I pushed myself through Peace Walker even though I hated it so that I could finally cross it off the backlog list.
I don't know if I fit your criteria of "hardcoreness" but lemme tell ya, there's lots of things to love in Metal Gear Solid's gameplay.
