That is not what happened. Two female assistants almost fell into that hole and notified of the dangerous state of the escalator but the people in charge kept the thing going.

There were no signs saying that there were problems with the escalator.

The mother rides the escalator with her son (who requested it) and she was notified at the last minute by the female employees that the floor was not safe so she grabs her son and this tragedy happened.
