This does sound really dodgy. If a PS4 disc is faulty, then it will fail at install. The PS4 wont install a disc it cannot read from so I'd be inclined to think it was something else. It might start to install, but if at any point it encounters DRE's, it will throw out an error code. Seeing as its not doing that, then the PS4 is reading the disc from beginning to end perfectly fine.
