1) always been like that
2) never existed
even in a perfectly competitive market... the only way on the long run to make a positive total profit is either to create a new market with a new product or differenciate a old one from the competitors.... or buy out the competition....
doing better products never warranted making more profit... actually it's probably the opposite as to make a better product you usually increase your total cost
