One of the comments said that the reason for there being three staff members at the top was that the escalator was closed for maintenance or something and the mother requested that they open it up for her anyways. The reason that they were so nonchalant about her falling through was that they expected it to happen.
Still an awful way to go though, and I feel terrible for the kid.
