Perma death has been around awhile, Steel Batallion on the original Xbox not only had perma death, it deleted your save file if you died.

im pretty sure most SRPGS have this. Final Fantasy Tactics, Suikoden. Hell Heavy Rain has it, even GTA one of your friends could die
