You choose your own coverage and it's still possible to wind up in over your head with medical bills even with insurance... Insurance and Health Care is Private in the US. ObamaCare basically just said the insurance companies can no longer turn people down for prior conditions, and that if you don't have insurance, you will be paying the government an annual fee to balance out the cost of yourself and other uninsured people.
There's no fixing it though, medical costs are absolutely absurd in the US and the whole "kick-back" system between the medical field and insurance companies is fixed in a way that you're utterly screwed if you aren't paying for insurance. It's incredibly easy for medical expenses to cross $100,000 for surprisingly little.
