Smoking has been in all three previous MGS games and it has had some practical uses (seeing laser beams if you haven't found the thermal goggles, cauterizing wounds or getting rid of leeches with the cigar in Snake Eater) but it also drains your health while you're using it and people will yell at you over codec if you're smoking. Heck, even in one of the trailers for MGS4, Sunny (the daughter of Olga) takes away Snakes cigarette, stomps on it and yells that it's a 'no smoking flight'.
The idea that nobody should smoke in a video game (especially one that's rated M) because of 'oh noes, teh childrenz!' just seems like bullshit to me.
