Uhm its psychiatric not really medical.
I really don't care that much about what it feels like to them. Similar to other psychatric illnesses such as bipolar disorder I don't try to understand the things they say as a result of their condition I just feel sorry for them and hope that they can find a way to restore their mental health.
Its the same thing as people who claim they feel like they've always been a wolf or a giraffe. The only difference is that in this case what they "feel" like they ought to be isn't quite as ridiculous to you as someone who feels like they should be a different species.
