First this isn't a strange occurrence this has happened before, and there was a time when those who did things such as this were considered saints. Technically you do not have to be mentally ill to do something like this. All it requires is a fervent desire. Perhaps it was a form of self decimation as a principle of faith, or perhaps she was self absorbed into a meditative state. One thing is for sure it wasn't so painful as to compel her to extricate herself.
Second bathing is not required for survival. That is correct you might stink like an open sewer, but that has no impact upon your health. Europeans used to go years without bathing, and walked in what amounted to open sewers in their bigger cities. Do you think they were in constant agony. Even some modern tribes in nomadic areas have individuals who have never bathed. The reality is you get used to it.
Third if this individual wanted to get off of that toilet they could in fact that might be why the police were called. The person in question might not want to damage their body while escaping, but no matter the atrophy adrenaline and mental overdrive would have that person rip the toilet out of the floor if they really wanted to free themselves. Any human being can summon that strength if the situation requires it. There are consequences later, but yes she could have gotten herself free.
Forth this isn't the husbands fault beyond enabling her. That however isn't a crime. Well at least in regards to adults. What interests me is how a human being could vanish for years and nobody notice. Had any of my neighbors gone missing. I think I would notice after at least a couple months.
