Rings any bell which was the ending it was present to the players in MGS2? But then she later appeared magically in MGS4, forcing us to rewrite whatever story we have founded 'till MGS1, and merely guessing that Snake just plain ignored her during MGS2.
Hell, even in one of the "Snake Tales" for the Substance version of MGS2 they acknowledge Meryl did die xD (though Snake Tales are non-canon).

They reference her, albeit fleetingly, in Metal Gear Solid 2. If you call Otacon enough times during the Tanker Chapter, Snake will make a few quips about having broken up with her (something about being over tomboys iirc).

Even after all these years, I keep discovering new secrets in the Tanker chapter o.O
I remember that if you saved enough times, Mei-Ling appeared instead of Otacon whenever you needed to save xD
