MGS4 assumes you have gone through the story that way.

Kinda like Dragon Age II? Have you played that one? The first have multiple scenarios. The second one, present you with a "version'' most players have made.

The point I am getting at, is, she's not mentioned EVEN ONCE during MGS2. That, and the fact that you're with Otacon...

 

Rings any bell which was the ending it was present to the players in MGS2? But then she later appeared magically in MGS4, forcing us to rewrite whatever story we have founded 'till MGS1, and merely guessing that Snake just plain ignored her during MGS2.

 

Hell, even in one of the "Snake Tales" for the Substance version of MGS2 they acknowledge Meryl did die xD (though Snake Tales are non-canon).

Yeah i know what you mean. It's just Kojima was really doing the Playstation some fanservice and was trying to bring as much elements back into the game as possible, you've played the game, right? Noticed all those flashbacks!!

It's a japanese thing in the end i guess, just like with the anime Naruto, they go with the story wherever they like every couple of seasons!! It's confusing if you're not used to it.
