Only thing I've seen is people who have no idea how to use the voice commands. You are supposed to pause for a brief second after saying Xbox, then the available commands come up on the screen in green letters then you say what you want it to do. Does it work perfectly every time? Of course it doesn't but people repeating it over and over obviously aren't saying the correct commands and/or not doing it correctly.
