Paying for a game that has xbl features only requires 2 purchases: 1.game 2.xbl

This is paying for a game that has xbl features that is roped off into another paywall. 3 purchases. 1.game 2.xbl 3.fitness  

I assumed based on your comment you were providing factual information, but upon further reading I'm not so sure.

From the reading for the web site, you get the five trainers for free as a part of the sub to Xbox Fitness.  This doesn't stop after December 2014.  Additional content and possibly trainers may become available later on that you can purchase.  So if a trainer you like comes out with new content, you can purchase it if you want but it isn't availble as a part of the base subscription.  Essentially DLC.
