I got warned once for puting an "offensive" signature (it was dark souls related). I found it unfair. The mod in question could have contacted me, tell me that I shouldn't put that signature (or removing the offensive part, which it is what I did), instead of going with a full-warning. In other case I put a signature that was off-limits, and Kantor warned me, but via wall-post (no full mod-warning thing), so that was cool.
So no, I don't have a perfect reputation, but because of unfairness :(
