Trophies are all cryptic. I think you gotta finish hard mode - but would that be just to pass it, or you gotta actually perform quite well or something?

Yeah, beat the game on hard. You just have to pass every song with at least a 'standard' rating. Not very challenging if you're familiar with rhythm games. There are a bunch of really cryptic trophies related to stuff that you would probably never think to do though. I would suggest looking at a trophy guide.
