The Big Boss that's missing an arm is the real boss. He most likely lost it in the attack on Mother Base. While Big Boss is in a coma somebody is posing as him. Theories state it may be Solidus.

I was hoping that wasn't true. That is a mistake. Big Boss didn't lose an arm. Not even the Metal Gear Encyclopedia acknowledges that.
