I think the most difficult part of survivor is when enemies are alerted to your presence. Since you have to beat the game at least once to unlock survivor, most of us have the encounters relatively fresh in our heads, so while having to play the game without listen mode was an adjustment, it was offset by my prior knowledge of areas.
However, when you've been spotted by the enemy, that is when I have noticed the difference in difficulty between hard and survivor. In my opinion, you get the most out of the game if you play a mixture of stealth (take out one or two enemies) and "guns blazing" (shoot the rest), because while stealth killing makes the game easier to progress through content, you're missing out on much of the spontaneity of the combat (e.g., shooting a fire extinguisher, holding someone at gunpoint, Ellie stabbing someone to get them off of you, etc.).
