i feel that joel saved ellie not for him self like you stated, but for ellie. he believes she doesnt deserve to die, and he promised to protect her. its clear he is carrying the burden of his decision with him. for example, when ellie ran away on the horses and he yelled at her he said "do you even know what your life means?" he knows how important saving ellie is and he knows how big her immunity is.
i believe he lied to her because he decided to carry the burden, he told her this is no cure so she can get it out of her head and so she doesnt feel that there is anything that she can do, why? so she can live her life, joel saw how much this girl is alive, how she isnt just surviving, and he believes that she DESERVES to LIVE her life. thats why he did what he did and lied, thats why he is carrying the burden of his HUGE decision with him and not telling her, so she doesnt, and so she can live.
also i think he knows that if ellie knew she would have to die to make a cure, she would sacrifice her self. i think that how much he cares about her and that he believes she deserves to live has made him not tell her.
and coming to think about it, omg is this such a good ending. unbelievable how deep it is.
