its weird i also had some fear in some levels (in the ghost level, the eating piano used to freak me out) though it was probably just because i was a kid. 

The ghost level was a little scary but I always found this ugly sea monster thing quite scary. 

that son of a bitch!!! you just reminded me of this guy lol. but later on (i played the game a billion times) i got used to the guy and he didnt freak me out

Really, I played the game 3 times (the first time I finished the game without discovering the flying cap!) but always got really scared getting near him. I think I had a few nightmares about him too :/
