This could be a crud buildup issue with your controller. Sometimes crud gets down in the valleys between the body of the controller and the buttons. In this case the crud could be causing one of the buttons to continually send off signals. Keeping your controller from fully synching up.
Get a sewing needle, or a thumbtack, and plumb the edges of each button on your controller. Paying particular attention to the back and select buttons. There is a reason most games don't use them, and it has everything to do with the fact that they get stuck easily. You might also want to use a wet rag to clean the surface in case the crud is really sticky.
