Shouldn't be that hard with how easy it is for them to track their Xbox Live stats. They can simply just give the amount of accounts that currently have Xbox Live Gold.

Another thing that I wonder is if the 46 million includes all accounts or just the one's that have been active somewhat recently (say within half a year or so)

Only active accounts that played at least once in the last 8 months.
