Things change during the final design phases, especially when dealing with custom CPU/GPU builds. Obviously they'll not be redesigning a board but there can easily be tweaks added late into the project as they'll have set milestone dates for when parts must be finalized for manufacturing which allow some room for change until as late as possible. No as far as I'm aware it did happen, although the RSX wasn't completely off the shelf. You might want to read up on the changes that happened to the Cell late on in development too as the original target was for a 4GHz frequency core (they hit 3.2). This had a knock on effect for other areas of the console design. Nothing is set in stone till very late on and most dev kits are low end estimations of the end product.
