What's the weirdest or worse glitch you have come across in a game?

[Soleron]

My 3DS out-of-box experience.

1. Insert Rayman.
2. Start game. Select first level.
3. Walk fowards.
4. Walk through wall
5. Fall into the infinite void

The 3DS didn't get turned back on for another 6 months.
