Weirdest: Infinite free throws in NBA Live '95 for the Genesis. We scored 1001 points and then took a pic! (I wonder where it is...).
Worse: Right now I remember the vanishing chests in God of War 2. In that game the content of some Health and Mana chests disappear when you advance too much. When I find a Health or Mana chest but I'm at 100% I save it for later, but when I come back the chest is open.
