It seems fairly straightforward. You write in a letter and the game makes suggestions of context-sensitive words. The first thing he writes is ムネ、　or "chest," and the game goes through remarking how flat the girl is. Later he writes "milk" and that leads to the girl trying to get at the milk.

Think of it like endless ocean. There's no point but seeing the things that are in the game-world happen.
