Metal Gear Solid 4 was not the most dissapointing as that dishonor goes to Metal Gear Solid 2 with its bait and switch nonsense with the main playable character, the introduction of FPS style shooting, gameplay that moved the series away from stealth, and a story and a cast of characters that were absurd even for Metal Gear.
In regards to examples that fit in with your idea ... Silent HIll 4 was the most dissapointing game in an otherwise perfect series when it was released. Umm ... that is all I got, but I am sure there are others.
I would also argue that Assassin's Creed Brotherhood was where things started to go wrong with the introduction of multiplayer. I would also say that it ruined the story, but that already happened in Assassin's Creed 2.
