1. Not being able to save anywhere. I hate it when a game tells me, all progress from previous check point will be lost if you quit now. Well where was that previous check point, where will I end up when I continue next time.

2. Sticky cover, nothing as frustrating as trying to doge and sticking to a wall instead right next to a grenade.

3. Context sensitive buttons that don't do what you expect. Especially in combination with 2. I want to jump out of the way, not crouch into cover.

4. A camera that you can't control or keeps doing it's own thing working against you.
Recent offender: Zelda SS, Dark souls

5. Directional controls changing based on camera view. Sometimes I wish I could control the character from the characters point of view again instead of figuring out what 2D movement to apply to a weird diagonal angle view from above or below.

6. Bad UI design.
Recent offenders: Most games have their problems but Skyrim stands out in the ridiculous amount of button presses you need to switch between spells and abilities in combat.

7. Unskippable drawn out heavily scripted boss battles, made worse with unskippable text and cut scenes
Recent offender: Zelda SS, I quit the game at Thunderhead.

8. Repeating / stating the obvious.
Recent offender: Zelda SS, I don't care that a red rupee is worth 20.

9. Long load times on failure. I don't mind hard games but I do mind having to wait for the entire level to reload that was already in memory.
Recent offender: Batman AC, wait to load the "haha you failed" cut scene, then wait to load the same level again, thanks.

10. Forcing you forward by triggering a cut scene which becomes the checkpoint, making it impossible to prepare for the fight ahead or turn back until you're ready.
