What TruckO said is pretty much my biggest pet peeve as well.
But to add something into the thread, developers thinking that someone is going to charge them money for every button they leave unassigned also somewhat annoys me. Just because you have the option of making a convoluted control scheme doesn't mean your game necessarily warrants one. (Not that I'm a fan of oversimplification)
