One at a time. Witcher 2's construction is kind of open world - but it's more like three sets of smaller open worlds, each with a bevy of quests you can handle whenever yo uwant. But if you progress the story too far, you go into the next hub and lose your opportunity to finish those quests.
It strikes a pretty decent balance, though I imagine this post has put you off buying the game.
