In order to appreciate MGS4, a number of steps must be taken before playing the game.
1. Play through MGS1,2, and & 3 (preferably at the times they came out, which isn't possible if you didn't already)
2. Understand--to your best degree--the overall plot
3. Play MGS4. At this point, if you enjoyed the rest of the games, MGS4 will be a good game no matter how much of the story doesn't make sense or how much there is to be disappointed with in general: you'll be enough of a fanboy to overlook those things :P
I personally loved the game, and it's definitely because I'm a fanboy.
Edit: I'd also like to say that the gameplay is great, which is the best aspect of a game imo.
