I would personally say Ninja Gaiden. While I do think some people in this thread are underrating Demon's Souls difficulty, it is still a challenging game, but a lot of the difficulty comes down to strategy more than skill. Ninja Gaiden requires combat mastery and quick reactions. Demon's Souls requires the same but to a lesser extent. To me, it is much more forgiving if you are talking about a first playthrough, especially with the plethora of healing items and the ability to summon other people. Now, if you are talking about higher difficulties, Ninja Gaiden easily takes the crown.
