It's possible to get sick from the proteins in semen itself. Another possibility is if he had some sort of infection...be it bacterial or viral in/around the head of the penis, urethra, foreskin, or (I'm assuming) hands, he could conceivably transport the microbes via ejaculation, leading to the woman becoming ill, after consuming the water. Imagine if he had a breakout of the infection at the time...his semen would have become mixed with the pus from the oozing pores and...gross. That's going to be in my head for a while.
To sum up, it's possible for one to get sick from semen.
