Yea, it isn't for sure but it is widely speculated that he either had syphilis, Parkinson's, or both. Just check out his wiki page and scroll down to 9 Health section. It even talks about the rumors that he only had one ball. I speculate that if he did then it would have been blown off during WW1 battle when he suffered an "groin" injury. There was also speculation that he had a sexual relationship with his half niece.
