Being too ambitious, taking too long to release (so that other games can make the previously cutting edge game look dated), rushing to release during a certain period, not finding bugs before a game launches, investing in a game that seemed like a good idea and releasing it anyway just to recoup some of the money spent on developing, not undertstanding the hardware it was made for, making a licensed game to release with a popular fad or movie release, tacking on motion controls to a game not intended for motion controls.
