If you touched the difficulty setting at all throughtout the game it will cancel it. If you start on normal and chance it to insanity as well it will not pop.

Yeah. I'm aware. I started my game on Insanity and never touched anything.

Man, now Im afraid. Im playing on Insanity. I hope I get the plat.

PS - The Suicide Mission music is awesome
