If it involves a large number of people in a world online at the same time, that is an MMO.  May not be an MMO RPG though, but it is still an MMO.  MMO stands for Massive Multiplayer Online.

I think the key might be persistent world.

Not sure about that.  Sandbox games will have forms of a persisent world.  Well, if the world goes on with other players in it, even when you are not, that also does lend to a game falling in the MMO camp.
