Once a year is a bit excessive. I don't even reapply TIM on my overclocked PC CPUs that often.
But if anyone is a heavy use player (say, 20 hours or more a week), it would probably be a good idea to clean it out once a year unless you have your PS3 running in a ventilated, dust-free bubble.
