I'm pretty sure that save wouldn't have worked. You're gonna have to start a new game to earn trophies, like in Uncharted 1 and GTA IV which also got post release trophy patches.

I don't know. Borderlands just released an update which supposedly fix a bug about trophies not being earned and providing said trophies using the save data, I think. So if such a blessing like a Trophies update for MGS4 could take place, maybe it could have worked.
