For something that was completely in their control to either do or not do, SCE's biggest mistake was not charging for PSN AT THE START. It would've helped the division make more profits and improved the features of the service to where it could TRULY compete with XBL.
As has been reported to some extent, though, blame SCEJ, as they still don't have the online vision that the West has embraced ...
