This is what they will do. I expect Bioware to have all of the decisions to be made which include all carry over players from the first game. (so the decisions would default to not killing people in the first game).
I dont think anyone would be alienated by seeing some new person in the universe without much explanation of who they are, if this is the case, then I am sure a new opening cinematic movie will recap every discrepency.
