In Modern Warfare 2 and Gears 1 & 2, I'd guess it's mostly connection based. That isn't to say there aren't some damn good players from other countries on these games, but I think it's easy to tell the difference. When three shots from your shotgun in Gears, or multiple hit markers in MW2 don't put your opponent down, but a single blast from them kills you...well that isn't skill based.
When that isn't the case and the same #$@* kills you five times in a row...well then regardless of country, you got owned. Sadly, I've been on both sides of that equation.
