It means Northerner. The term Yankee was used by the South to differentiate themselves from the North during the Civil War... and often still used to this day to highlight the cultural differences between the North and the South.
It's the equivlent of calling someone from England or Northern Ireland "Scottish".
Well except a lot of the South still dislikes the North quite a bit.
Southerners may find it insulting because it'd be like calling some prissy or metrosexual or something.

Ok, but for anyone born in America like me, saying American as a demonym for USA is also insulting towards the rest of the continent, what other word is there? Usaian?

I have to say I heard enough times Yanks saying "I'm an American" and such like. If Americans say it you can't have ago at the rest of the world for following suit.

I'm not saying it's "wrong", all I'm saying is that I'm certainly not following suit...until I find a better word, yankee is an excellent replacement. Plus, let's be serious, no one except anyone from the US will think that Yankee means anything besides...well...born in the US
