You don't have to fit a strict set of rules to be an rpg, and no, you don't need towns to be an RPG. To be honest the bare minimum is just a few RPG elements that recur very often in your game is enough. Look at Borderlands for example.
Pen and Paper Role Playing games are still rpgs despite the lack of towns or story.
