I'm sure the game could have an optional 3D mode and wouldn't need to be on a different copy. Especially with Blu-ray there should be more than enough space, even if they needed to include a second 3D engine for stereoscopic output (which I think is unlikely).
The new Samsung 46inch 3D LED is listed with US$2600 right now (here - afaik it's the first of the TVs that comply with the 3D standards), so I guess by this summer 40 inch 3D TVs (non-LED) could start at ~US$1500-1800

yes, stereoscopic 3D theoretically can work at 100Hz as well
