To keep Shepard alive, the fewest possible survivors you can have is two. On one of my playthroughs I intentionally made Tali the sole survivor, but my stupid Shepard died on me regardless.
That YouTube vid I posted earlier just made it seem like Tali was the only one left alive, but there really was one other survivor (Samara just had the courtesy of not stealing Tali's limelight).
